In-Clinic Appointment Reservation Policy
Clinic Appointment Reservation Fee - Effective 6/23/2025
We now require a $30 appointment reservation fee for all In-Clinic/In-Person appointments.
Here are the steps once you request an appointment:
- The fee will be collected, via Zelle or CashApp, at the time we receive your request for an appointment.
- Our staff will verify that the fee has been received BEFORE confirming your appointment.
- Once payment is received, your appointment will be approved by a staff member and you will receive a message from our EHR and a link to complete the appropriate Intake Forms prior to your appointment.
Refundable Fee if:
- you check-in for your appointment on the scheduled date within 15 minutes of your scheduled appointment time
Non-Refundable Fee if:
- you cancel less than 13 hours in advance.
- you do not show up for your scheduled appointment.
Patients with Employer Paid Pre-Employment/DOT Physicals/Drug Testing
The fee is required and is the responsibility of the patient even if your employer is paying for your visit. Once you arrive for your scheduled appointment, the fee will be immediately refunded.

Appointments are available on a first-come first-served basis and we make every effort to be available to our patients. We understand life happens, and appointment cancellations are often inevitable. Our providers and staff seek to be good stewards of our patient's time and we ask that our patients do the same.
Arriving Late for Appointments
Telemedicine Appointments - please log in for your appointment at least 10 minutes before your appointment time. If you are more than 10 minutes late, your appointment will be cancelled and you will need to reschedule your appointment. See appointment cancellation policy below.
In-Person Clinic Appointments - if you are more than 15 minutes late, your appointment will be cancelled and you will need to reschedule. You may call the office and attempt to reschedule your appointment for the same day pending availability. See appointment cancellation policy below.
Appointment Cancellations
You must cancel your appointment 12 hours before the appointment date and time to avoid a $30 cancellation/no show fee.
Same Day Appointment Booking and Cancellations
For your convenience, we do allow same-day appointment booking up to 2 hours before the requested appointment time. If you book and decide you need to cancel the appointment, or you do not show up for your appointment, you will incur a $30 cancellation /no show fee.
